We want to focus on your treatment during the time we have set aside for you. To help, we have forms available to you on-line so that you can complete them prior to your visit. We would prefer that you mail them to us but you may bring them with you to your visit. Whether you have had the opportunity to fill out the forms in advance or not, please come with the following information:
1. The date your last menstrual period started. 2. Any medications or supplements you are currently taking and their doses. 3. Any drug allergies you may have. 4. Your current insurance card for verification of eligibility.
Your appointment begins when you arrive - and we will spend the first few minutes updating your records in our system, and then will call you back to begin your exam.
Insurance
As insurance policies and guidelines are always changing and vary from plan to plan, it's a good idea to check with your insurance company before your visit to be sure:
1. Your plan lists Dr. Johnson as a provider 2. You know what your copayment for the visit will be 3. Your plan will cover the services you need 4. You have any referral required by your plan, as Dr. Johnson is not a PCP. COPAYMENTS OR DEPOSITS ARE DUE AT THE TIME OF SERVICE.
Forms, Cancellation Policy, and When To Arrive
Please arrive on time. Your appointment begins with the completion of any necessary paperwork and the updating of your chart. When you are on time, this gives us time to update your records and bring you back into the exam room on time for your appointment. Again, we will try to mail the forms to you in advance and to make the forms available on the website so you can return them to us before your appointment and reduce your waiting time.
If you have never seen Dr. Johnson before we will be creating a new chart for you. You will need to fill out a Patient Medical History and Registration Form, as well as reviewing and signing our Credit Policy, Patient Responsibility Form, and Privacy Notice, even if you have records transferred from another clinic.
If you are an established patient at our current location, we will probably ask you to fill out a form regarding Today's Visit. Be sure to let us know if any of your medications, insurance or other relevant information in your chart has changed.
We do our best to see every patient on time, and understand that your time is valuable. Occasionally, however, there are emergencies that arise or someone in need of extra time that put us behind. We will do our best to let you know when this happens, and ask for your patience. Someday, if you need a little extra time, we will do our best to accommodate you.
Please let us know 24 hours in advance if you will not be able to make your scheduled appointment so that we can make the time available for another patient. If you fail to show for 2 apppointments without notice, you will be responsible for a rescheduling fee before another appointment can be made.
When You Arrive
Dr. Johnson's practice shares space with The Oregon Clinic, Surgical Division. When you arrive and check in, members of both staffs will be at the reception desk, and only a member of Dr. Johnson's staff will be able to check you in. Dr. Johnson's Medical Assistant will be out soon to welcome you and bring you back for your appointment. We look forward to seeing you!
Preparing for Your Hospital Stay
Dr. Johnson is on staff at several area hospitals, including Legacy Good Samaritan Hospital, Legacy Emanuel Hospital, and Providence Medical Center.
Whether you're going in for a simple day procedure, or will be staying overnight for surgery, it can helpful to have additional information about the facility.
